The first grade is for the sleeve and second grade is for the media/disc, e.g. VG+/EX means that the item's sleeve is in Very Good Plus condition and the media/disc is in Excellent condition. If there is only one condition, e.g. /EX, it means that the item is not in a picture sleeve and that the media/disc is in excellent condition. Records that aren't in picture sleeves will either be in a company sleeve or a generic plain sleeve.
